What is going on with RTM? Subtasks are appearing in-line

For the last couple of mornings, I've found sub-tasks of main tasks inline with the main tasks. Yesterday it was a couple. Today is was five. Also, the notes on the sub-tasks are just *gone*.
I had to re-enter the sub-tasks under the main tasks but have no way of getting the notes back.
Someone needs to look at this immediately.
